Annual Student Declaration
To continue coverage on your extended health and/or dental plan beyond age 21 and up to the maximum age on your plan.
Employee Change Form
Use this form to update a covered employee’s information. Examples: Add new family members, change names, update salary, terminate coverage, etc.
Amendment to Coverage Status
If your health or dental coverage situation changed—either declining because you now have another plan (like a spouse’s), or enrolling because prior coverage ended—this form updates your status.
Employee Enrolment
Use this to manage life insurance beneficiaries: adding more than three at enrolment, or updating and replacing an existing beneficiary designation.
